Step Down Community Care
IDG Care provides community-based step-down placements which support the move of individuals from inpatient services back to the community.
The service facilitates early discharge from hospital, reduces the length of stay on acute mental health wards and enables people to reintegrate back into the community in a safe and supported way.
IDG Care offers 24-hour live-in care, with in-reach from a community team made up of a nurse, social worker and support worker. The service takes an innovative approach to supporting patients who could benefit from continuing care in a community based, recovery-focused environment. Residents are supported with activities of daily living and any social or practical needs ensuring safe social integration and management of requirements particularly from the Ministry of Justice (MOJ) or hospital e.g. Community Treatment Orders (CTO).
